It apparently occurred in shipping, although the box looked pretty good. Apparently the shipping had rattled the light hard enough to make components come loose. I re-assembled the fan but, it didn't work. I contacted the seller and suggested I could try to install a different motor. He agreed and shipped one right out. After installation, I found the fan still did not work. The seller refunded, no questions. GREAT SSELLER!

Awesome communication, item took over a week to get here, not sellers fault, USPS was totally useless. Parcel was tracked all over the country, almost making it back to seller, before they get it right. Stick with UPS, they have a much better service that makes sure items get to where they are supposed to be. Once again. NOT sellers fault.
